A medical release/permission form is required for all out-of-town and off-site Student Retreats and trips. Click here to download this form. Please fill it out completely and return it to Damon Stalvey. Because this form is a permission form as well as a medical release form, students will need to fill out a new one each time they participate in an off-site Youth event.
